Service Animal Letter

A Service Animal Letter provides official confirmation from a licensed U.S.-based healthcare provider that your animal has been trained to perform tasks that assist with your disability. Unlike an Emotional Support Animal, a Service Animal is granted broader access rights under the Americans with Disabilities Act (ADA), including the ability to accompany you in public spaces and workplaces where animals are typically not allowed.

After a brief evaluation, your provider will issue a personalized Service Animal Letter including their license number, signature, and required legal language verifying your need for a trained service animal. Your privacy is protected — your specific diagnosis will not be shared unless you request it.

This letter can be used for:

  • Housing and landlord accommodation requests
  • Workplace access considerations
  • Travel, public spaces, and service animal legitimacy clarification
  • Reducing conflicts when your service animal accompanies you
